Report Criteria
|
Increase flexability with multi-level reporting capabilities (e.g., identify patients
missing critical tests, patients who fall outside the evidence based target range;
by payer; for provider, site or group; etc.).
|
Registry Summary Reports

Provide your practice with population based (aggregated) process and outcome data
on all patients with a specific condition. Track demographics (numbers and percentages
of patient visits, genders, ages, insurance types, etc.), visit info (blood pressure,
medications, specialty care, etc.), and test info (for Diabetes HbA1c or Glvcosvlated,
cholesterol test, LDL test, etc. and for asthma FVC, FEV1, Peak Flow, etc.).

With imbedded evidence based guidelines, designed by physicians to mirror the office
visit, Page 1 of the Diabetes Progress Note reminds physicians and patients about
needed care services (e.g., eye exams, foot checks, patient education and specialty
care) and prompts the ordering and retrieval of tests and screens (e.g., HbA1c,
LDL, etc.).

The colorful Page 2 of the Diabetes Progress Note communicates to patients their
personal clinical trend data and its relevance.
