Touch Every Life © (TEL) is a chronic care registry with specifications to improve
diabetes and asthma care. TEL provides the information support needed to implement
elements of the chronic care model in order to improve patient care.
The registry was initially piloted in an internal medicine private practice, and
enhancements have been made to facilitate workflow within the offices, including
the ability to interface with a central lab to import data electronically. The flow
sheet allows for evidence based guidelines to be embedded in the care process and
was designed to mirror rather than dictate the flow of the patient/physician interaction.
As payers and employers look to healthcare providers to implement systems of care
that provide improved clinical outcomes, TEL has provided the means of moving in
that direction. We are hopeful that TEL will give you a “jump-start” in this process,
putting you ahead of the curve as payers present opportunities relative to reimbursement
and as coalition sponsored quality scoring becomes more prevalent.
We plan to expand the features of TEL to allow for its use with other chronic illnesses.
Benefit from the rich capabilities of TEL, including:
- Notify patients who are overdue for office visits.
- Remind physicians and patients about needed care services (e.g. eye exams, foot
checks, patient education, and specialty care).
- Prompt the order and retrieval of tests and screens (e.g. HbA1c, LDL).
- Track prescription of medications by class (e.g. ACE, ARB, lipid agents) for the
conditions.
- Track and follow-up on test results which are not at goal.
- Communicate to patients, personal clinical trend data and its relevance.
- Track and follow up on referrals to relevant specialists for this condition
- Track your patient population with this condition.
- Provide your practice with population based (aggregated) process and outcome data
on all patients with a specific condition.
- Compare process and outcome measures within network, group and site to identify
best practices.
